SUMMARY AS PASSED:

Hospital discharge procedures; designation of individual to receive information and instructions. Requires hospitals to provide each patient admitted as an inpatient or his legal guardian the opportunity to designate an individual who will care for or assist the patient in his residence following discharge from the hospital and to whom the hospital shall provide information regarding the patient's discharge plan and any follow-up care, treatment, and services that the patient may require. This bill is identical to SB 851.

Hospital discharge procedures; designation of individual to receive information and instructions. Requires hospitals to provide patients or their legal guardians the opportunity to designate an individual who will care for or assist the patient in his residence following discharge from the hospital to whom the hospital shall provide information regarding the patient's discharge plan and any follow-up care, treatment, and services that the patient may require.

Hospital discharge procedures; designation of individual to receive information and instructions. Requires hospitals to provide patients with the opportunity to designate an individual to receive information about the patient's discharge plan, together with information and instruction about any follow-up care, treatment, or services that the individual may require, and requires hospitals to provide a discharge plan and information regarding follow-up care, treatment, and services to the designated individual prior to the patient's discharge or transfer to another facility.
