SUMMARY:

Voluntary Formulary; narrow therapeutic index drugs. Prohibits the substitution or interchange of any narrow therapeutic index drug without the documented consent of the patient's prescriber as provided in regulations of the Board of Pharmacy. Narrow therapeutic index drugs are those pharmaceuticals that exhibit narrow ranges between the benefit and the risk of the drug. Narrow therapeutic index drugs include: carbamazepine (epilepsy), digoxin (heart stimulant), levothyroxine (thyroid deficiency), phenytoin (anticonvulsant), theophylline sustained release (smooth muscle relaxant), and warfarin (anticoagulant).
