SUMMARY:
Recordation information form. Allows the circuit court clerk to require the filing of a recordation information form with any writing presented to the clerk for recordation. The form is to be used for indexing and recordation tax purposes, but will not be considered a part of the filing for purposes of assessing filing fees.
