SUMMARY:

Accident and sickness insurance; calculation of cost-sharing provisions; explanation of benefits. Applies requirements of insurers, health services plans and health maintenance organizations to (i) calculate cost-sharing provisions based upon the total amount actually paid to the provider and (ii) file explanation of benefits forms, to insureds, subscribers and enrollees who are residents of Virginia or whose claims were processed by or for insurers, health services plans and HMOs within the Commonwealth.
