The notice shall (i) state that a constitutional amendment will be on the ballot and the date of the election, (ii) contain the 500-word or shorter neutral statement describing the amendment prepared pursuant to this section, and (iii) state that the full text of the amendment and the ballot question may be obtained from the State Board of Elections' website address (shown in the notice) or at the office of the local general registrar.
