SUMMARY AS PASSED:
Adoption; records of child. Provides that the investigative report that must be made to the circuit court shall include a statement by the child-placing agency or local director of social services that all reasonably attainable background, medical and psychological records of the child have been provided to the prospective adoptive parents, a list of the records provided, and the relevant physical and mental history of the birth parents if known.
SUMMARY AS PASSED HOUSE:
Exchange of medical and biological information for adoption; affidavit. Requires the filing of an affidavit stating that all reasonably attainable medical and psychological records of the child were provided to the prospective adoptive parent(s) prior to the entry of a final order of adoption. The affidavit shall be signed, under oath, by the agency or parent(s) and the prospective adoptive parent(s). The affidavit shall also include a statement of which documents were provided.
