SUMMARY AS INTRODUCED:
Health; vital statistics. Requires the Department of Health to utilize 1 uniform form of birth certificate that has a space for the names of both the father and the mother. If the father is unknown, the space shall be left blank. Currently, the state uses 2 birth certificates, 1 for a birth where the parents are married, and 1 for births to unmarried persons. The bill also allows a father who voluntarily acknowledges paternity to have his name placed on the birth certificate without the consent of the mother. The bill also provides that a person who voluntarily acknowledges paternity and is paying child support shall have his name on the birth certificate.
